A Case Manager really is the glue in a person’s treatment plan and recovery.
Our case managers connect clients to a wide range of resources and coordinate with each member of a person’s treatment team to ensure clients receive the proper treatment according to the treatment plan. Case managers also provide therapeutic and connective services like discharge planning, housing assistance and continued care after hospitalization. Case managers utilize a variety of treatment approaches to work with a diverse population including children, adults, individuals, couples, and groups.
Mobile Recovery Team: The Mobile recovery team works with community partners to engage and integrate stops on the recovery continuum, including primary care practices, peer groups, and other social service agencies serving at risk populations. Through community building efforts, the team will have an opportunity to engage in motivational interviewing techniques with County residents who are struggling with substance abuse and addiction.
Bachelor’s degree,preferably in human services or related field, required
Previous experience with substance abuse and mental health-related case management and treatment required
Current valid Colorado Driver’s License required
Certified Addiction Counselor II or III is strongly preferred
Minimum of two (2) years human service-related experience preferred
