Works with a designated clinic care catchment area and physical health care providers to provide comprehensive clinical assessment/problem identification, crisis intervention, individual, family and group therapy and case management. Including home visits and support for identified high-need patients. Develops and monitors treatment plans and progress, documents significant interactions, and maintains clinical charts and other reporting data. Works with a diverse client population in a variety of treatment approaches and/ or setting. This position also provides health promotion to patients and serves as a health promotion instructor. This position also provides periodic staffing support for outreach events.
Master’s degree in human services or related field required; Two (2) years previous experience in human services related field preferred. Certified Addictions Counselor (CAC) designation or currently pursuing designation, depending upon program and as needed. Applicant must be license eligible and receive license within eighteen months of hire.
